Question, can the turbo be rotated by 180 degrees?
On my first engine from a E300D (W210) the output was at the bottom, on my newer engine (source unknown) the output is at the top and it fouls the body.

Yes I have done this, you just have to change the mount for the wastegate actuator.

Maybe a silly question but how? The oil feed comes off the top, does that part stay and you rotate the impeller body?
Otherwise i'd have to modify the oil feed too

The CHRA (center-housing-rotating-assembly) contains the oil/coolant inlet/outlets along with the shaft+turbine+compressor wheels. The CHRA needs to mounted with the oil drain pointing nearly straight down. 

The compressor housing and turbine housing attach to the CHRA and can usually be rotated in various degrees. Some have a bolt pattern that allows for 90-60 degree increments. Some clamp against the CHRA and can be rotated to any angle.

Where you can run into issues with an internally wastegated turbo is how the wastegate actuator attaches to the turbo. The actuator needs to maintain the same orientation with the turbine housing. If the mounting bracket for the actuator is built into the compressor housing, then it will limit your ability to rotate the housings. If the wastegate actuator attaches to a bracket separate from the compressor housing, then you should be able to loosen and rotate the compressor housing.

Thanks Vica, in fact i need to make a new wastegate bracket anyway as the previous guy got part way through converting to pressure actuator and i dont have the original bracket.
Theres no issue unbolting the compressor? the CHRA bearing dont fall out or anything?, sorry never disassembled turbo before.
I think its been rotated by the other guy anyway, i dont see any standard OM606 where the output is at the top.

Can someone give me a clue how to move the snail around by 180 deg? As mentioned, never done it before need some pointers.

Backside of the snail should have 4-6 bolts securing it to the front cast iron lip of the CHRA (center housing rotating assembly).

Loosen, do not remove, these bolts and rotate the snail to the desired position.
Re-tighten the bolts.
